To request an account, submit your request to the Sky Lakes clinic where you receive services. By requesting a Sky Lakes MyChart account, you agree to the Terms and Conditions of Use described below.
We think it is important for you to know how we handle information we communicate via Sky Lakes MyChart. This Terms and Conditions of Use outlines our practices and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason. We may modify these Terms and Conditions at any time. Any modifications made to these Terms and Conditions will be effective immediately. Any aspect of Sky Lakes MyChart may be changed, supplemented, deleted or updated with or without notice at the sole discretion of Sky Lakes.
Response to Electronic Communication Ė Sky Lakes will use its best efforts to provide a timely response to electronic inquiries. Because the clinic staff who may be required to respond to your inquiry may not be immediately available, you should allow at least three (3) business days for a response. Emergency situations requiring immediate attention should not be submitted electronically.
With respect to any electronic communications sent by you, we are only able to respond to such communications based on the information you have provided. If there is insufficient information provided, we may be unable to provide accurate and reliable services.
Web Site Links - Sky Lakes MyChart may offer links to related medical Web sites not managed by Sky Lakes. These Web site link(s) are for your informational purposes only. Sky Lakes does not endorse and has not verified the accuracy of the information in/on these Web sites.
Minor Eligibility - You may request a Sky Lakes MyChart account if you are 14 years of age or older. Parents or guardians of minor children may request a Sky Lakes MyChart account on behalf of their minor children as well as access to their minor child's account by following the terms outlined in the "Proxy Access" section below. Under State and Federal laws, there are certain types of medical information that the parent or guardian of a minor patient may not view without authorization of the minor patient.
Proxy Access - Parents or guardians of minor children and individuals age 18 or older may request access to another individualís Sky Lakes MyChart account by completing the Sky Lakes MyChart Proxy Access Request Form and submitting it to the Sky Lakes clinic where they receive services.
A Sky Lakes MyChart account will be activated for both the proxy and the account holder. If the proxyís legal relationship with the account holder changes, the proxy is required to inform the clinic immediately. Sky Lakes reserves the right to revoke proxy access at any time for any reason.
E-mail Privacy - Patients who are users of Sky Lakes MyChart should be aware that they will be notified via e-mail when there is new medical information to be viewed in Sky Lakes MyChart. This means that any person with access to a patientís e-mail will be able to see this notification. This could include the patientís spouse, employer or anyone else that can access a patientís e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing Sky Lakes MyChart may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address.
Your confidential medical information in Sky Lakes MyChart will be accessible to staff as necessary to complete the functions of their job. Please know that if you send us an e-mail communication, it may be shared with any Sky Lakes staff that assists the physician in providing the patientís medical care.
Security and Confidentiality - Sky Lakes is committed to protecting the confidentiality of your medical information. We afford the same degree of confidentiality to medical information stored in Sky Lakes MyChart as is given to medical information stored by Sky Lakes in any other medium. We limit Sky Lakes employeesí access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We keep a record of all access to Sky Lakes MyChart, including the date and time of access by a patient, proxy, or Sky Lakes staff. We will take all appropriate steps to ensure information received from our online visitors is as secure as possible against unauthorized access and use.
As a Sky Lakes MyChart user, your role in maintaining the security of your medical information is: 1) choosing a strong password and changing it on a regular basis, and 2) keeping your login ID and password confidential.
